For Executive Directors & Directors of Nursing

On-site therapy solutions for Senior Living Communities

We bring outpatient physical, occupational, and speech therapy into your building — with our own equipment, our own therapists, and a program built around how your community actually runs.

Why Communities Partner With Us

Therapy that supports your operational goals AND your residents

Strong on-site rehabilitation touches the metrics your ownership group already tracks: resident retention, length of stay, incident reports, and family satisfaction.

Fall risk

Early identification through recurring screenings, paired with targeted balance and gait intervention before an incident occurs.

Hospital readmissions

Post-acute follow-through inside the community, so recovery continues after discharge instead of stalling.

Length of stay & retention

Residents who maintain function stay in your level of care longer, rather than moving to a higher-acuity setting elsewhere.

A note on outcomes data: we describe what our programs are designed to do rather than advertising specific reduction percentages. Published outcome statistics should be traceable to your own community’s before-and-after data — which is exactly what we’ll help you measure once we’re working together.

Core Value Pillars

What we bring into the building

New move-in functional assessments

A baseline safety check in the first days after a resident arrives — when unfamiliar layouts, new medications, and the stress of moving make fall risk highest.

Seamless DME solutions

We partner with established durable medical equipment providers to source, fit, and deliver adaptive equipment quickly, with immediate training for your staff and the resident.

Comprehensive fall risk screenings

Recurring quarterly mobility screenings designed to surface physical decline early, while it is still straightforward to address.

Interdisciplinary collaboration

Our therapists participate in your care plan meetings and family consultations, so therapy goals and community goals stay aligned.

Staff education & in-services

Practical training on Hoyer lifts, sit-to-stand transfers, body mechanics, and dementia care strategies — delivered on your floor, on your schedule.

Getting Started

Four steps from first call to ongoing care

  1. Facility evaluation

We walk the building with you — available space, resident population, current therapy arrangements, and the gaps your team is already feeling.

  2. On-site setup

We install therapy gym equipment in the space we’ve agreed on and establish scheduling, documentation, and communication workflows with your staff.

  3. Resident screenings

Baseline functional and safety screenings identify who would benefit from evaluation, and we coordinate scripts with each resident’s physician.

  4. Ongoing on-site care

Regular one-on-one therapy, quarterly re-screenings, care plan meeting participation, and recurring staff in-services.
