What a session with us looks like

Dedicated 100% one-on-one sessions

Your therapy time is yours. One licensed therapist, one resident, for the whole session — never split between two people or supervised from across the room.

Specialized gym equipment on-site

Real therapy equipment inside the community means we can actually challenge strength and balance, rather than working around whatever furniture is available.

Customized independent exercise plans

Every resident gets a written plan for the days between visits, sized to what they can safely do on their own.

Transparent communication

We keep family caregivers and community nurses informed about goals, progress, and any concerns we notice along the way.

Getting Started

Four steps to beginning care

  1. Reach out

    Speak with the community staff or the resident’s physician — or contact us directly and we’ll help coordinate from there.

  2. Align Motion evaluation

    A licensed therapist completes a full functional evaluation on-site, usually within 24–48 hours of receiving the script.

  3. Customized care plan

    We set specific, meaningful goals with the resident and family, and certify the plan with the physician.

  4. Continuous progress tracking

    Regular one-on-one sessions, an independent exercise plan for between visits, and ongoing updates to family and community staff.
