This policy describes how Align Motion Physical Therapy(“Align Motion,” “we,” “us”) handles information collected through this website. Protected health information submitted through the physician referral portal is additionally governed by our Notice of Privacy Practices.

Information we collect

• Contact and callback requests: name, email, phone, the role you select, and any message you write.
• Partnership inquiries: community name, your name and title, contact details, bed count, and preferred start window.
• Employment applications: name, contact details, discipline, license number, and any resume you attach.
• Physician referrals: referring provider details and patient information including name, date of birth, community, room number, diagnosis, clinical notes, and any prescription file attached. This is protected health information.
• Technical data: IP address and browser user agent, recorded for security, abuse prevention, and access auditing.

How we use it

We use submitted information solely to respond to your inquiry, evaluate an application, or coordinate and deliver therapy services. We do not sell information, and we do not use it for advertising or profiling.

How we protect it

• All traffic to this site is encrypted with TLS.
• Patient information in referrals is encrypted at rest using AES-256-GCM before it is written to storage.
• Uploaded prescription files and resumes are stored encrypted, outside any publicly served directory, and are retrievable only through an authenticated staff route.
• Every access to a referral record is written to an append-only audit log recording who accessed it, when, and from what address.
• Notification emails about referrals deliberately contain no patient information.

Retention

Referral records are retained for [[ RETENTION PERIOD — confirm with counsel ]] and then purged automatically. Contact, partnership, and application records are retained only as long as needed for the purpose they were submitted for.

Cookies

The public marketing site sets no advertising or analytics cookies. The staff console sets a session cookie that is strictly necessary for authentication.
